By purchasing or registering for any Haus of Glass Dance classes or memberships you will agree to the following terms. These terms will be presented at the time of registration and signed:

Medical

I understand that dancing is a strenuous physical activity. To my knowledge, the student has no medical problems or injuries that would prevent participation in class. I understand that it is my responsibility to inform a physician and to obtain written consent for participation in dance classes. I agree to inform the instructor(s) of any known health condition that may affect class participation. I agree to release Haus of Glass Dance LLC, its participants, or any instructor from liability related to accidents, injury,and/or illness.  I also agree to communicate in the event I have any signs of illness, or an injury, and understand that physical clearance from a doctor may be required to return to class. 

Credit Card/Bank Account Agreement

By registering and paying for Haus of Glass Dance  classes, events, services, or products I hereby authorize Haus of Glass Dance  to charge my credit card or bank account in advance for all applicable fees incurred by me in connection with my chosen service and registration. I hereby understand and agree that Haus of Glass Dance LLC  will be charging my designated credit card or bank account in accordance with the payment schedule of the service for which I have registered, but some service fees may accumulate on my credit card account or bank account before they are actually charged to my account. I further understand and agree that it is my responsibility to notify  Haus of Glass Dance of any changes to my credit card or bank account or if my credit card has expired otherwise my access to the Service may be discontinued or interrupted and fees will apply. All fees shall be paid in U.S. dollars. 

Tuition, Registration and Cancellation

I understand and agree to the following:

  • Tuition for our Refresh 2022-2023 Season is based on two different pricing structures depending on dancer age. Curriculum Classes for ages 2-17 and Memberships for teen/adult combined classes and adult only classes. By signing this agreement you are authorizing being enrolled monthly autopay billed on 1st of the month. Tuition breakdowns can be found on our website under “Tuition” and in your studio handbook.

  • Tuition is subject to change.

  • Tuition is non-refundable once the season has begun. Registration Fees are non-refundable. We do not prorate or refund for absences, but we do offer free make-up class options or discounted make-up private lessons.

  • All registrations are subject to a one time annual $25 registration fee for both new students and recurring students.

  • For those who cannot commit to a full term, we offer a drop-in rate of $25 a class. Amount must be paid prior to class on our website or billed to your account. 

  • Curriculum classes are for students ages 2-17 and these students are committing to the full season August 22nd 2022 - June 10th 2023. All of our curriculum classes perform in our Spring Recital in June of 2023. Dancers who are unable to perform in our recital must notify Haus of Glass Dance February 28th 2023.. Dancers performing in recital agree to be responsible for paying for recital tickets and performance fee outlined in section Costume/Recital.

  • All classes: As a general rule, if a student no longer wishes to attend a class, an email must be sent to Haus of Glass Dance 1 month in advance to avoid being charged for the following month. For ex: If you want to stop attending classes in October, notice must be made by September 1st, otherwise October tuition will be billed and your registration will end starting November. Haus of Glass Dance does not offer refunds if tuition for the next month has already been processed and if communication has not been confirmed 1 month in advance.

Enrollment 

Haus of Glass Dance automatically enrolls Fall Semester students for the Spring Semester. Please send an email to Haus of Glass Dance by December 15th if you or your dancer will not be participating in the Spring Semester. Please Note: Haus of Glass Dance reserves the right to refuse or terminate enrollment for any reason. No refund of the current month’s tuition will be made upon termination of the agreement; however, future monthly automatic payments will cease upon notification. New students will have the opportunity to enroll for the Spring Semester starting in December.

Dress Code 

I understand that dance requires proper attire for the dance class setting and the student must be dressed appropriately for class(es) as outlined in the class attire section on registration portal. I am aware these are costs in addition to the regular monthly tuition and that the student may be asked to only observe class, if not properly attired. Required dress code information for each discipline can be found in your studio handbook. 

Costume / Recital Agreement 

I understand and agree to the following:

  • Costuming is required for Haus of Glass Dance Spring Recital. 

  • All dancers participating in recital are billed on March 1st 2023 based on the # of dances they are performing in. 

Performance Fee Breakdown:

  • 1 Class - $120

  • 2 Classes - $215

  • 3 Classes - $300

  • 4 or more (Unlimited) - $375

Parents/dancers will be billed their performance fee on March 1st which is auto charged with the March tuition fee. I also understand that some self-costuming is sometimes required and details will be provided by your instructor and Haus of Glass Dance with dates by which attire must be purchased by. (Payment plans for Performance Fee are available upon request.) Purchasing tickets is required to watch the recital. This payment covers the cost of theater rentals, printing fees, staff to run the show, and lighting.

Schedule Changes 

I understand that a class could be canceled, or the day/time changed, if it does not meet the minimum requirement of four participants. I’m also aware that if a class is canceled or the day/time changed, alternative opportunities will be provided in an effort to accommodate. 

Trial Week 

You’r free “Trial” class happens the first week of Fall and Spring sessions only. This provides students the opportunity to experience a class without obligation to a drop-in or monthly fee during that week. The student must register as a Trial in order to receive the class(es) for free.

After the trial week, if the student decides to continue for the full session the full monthly tuition will be charged, and the obligations outlined in this Agreement apply. 

NOTE: If you are currently in our system, the software will not allow you to register as a “Trial”. Please email Haus of Glass Dance and we will enroll you.

Attendance 

I understand that refunds are not given for missed classes and tuition applies regardless of absence. I understand that attendance is important for the student and their classmates to optimize their learning. I’m aware that dress rehearsal and attendance two weeks prior to a recital are mandatory and will ensure students are present. I agree to schedule a private lesson(s) in the event the student misses more than three classes and is struggling to learn the technique and/or choreography for performances. 

Billing 

  • I understand that Haus of Glass Dance collects tuition monthly through automatic withdrawal (from a debit or credit card) on the 1st of the month, unless the balance is paid in advance or full payment for the entire season is made.

  • Registrations after the first of the month will need to make payment the day of enrollment and your tuition will be prorated. Haus of Glass Dance can accept checks and cash for monthly, semester, or yearly payment. You will need to call or email Haus of Glass Dance register for classes using cash or check.

  • Late payments will result in a $25 charge. A payment is considered “late” if it is not made within 1 business week (5 days)  of the due date. A secondary Late Fee will be charged if tuition is not paid by the 15th of the month. Tuition will be sent to collections if it remains unpaid without communication after 30 days.

Holidays/School Closures/Canceled Classes

  • I understand that Haus of Glass Dance generally follows the Grand Rapids Public School schedule. We are CLOSED for the following holidays/breaks: Labor Day, Thanksgiving weekend, Christmas break, Spring break, Memorial Day and a portion of Summer break. Tuition will not be prorated on months that have closings for holidays.

  • In the event of a district-wide school closure, Haus of Glass Dance  will consider following the district’s lead. If Haus of Glass Dance decides to follow suit, to the extent possible, the Haus of Glass Dance website and social media accounts will reflect the closure and an email notification will be sent. In the event of classes being cancelled due to poor weather or teacher absence (if a substitute can’t be provided), Haus of Glass Dance will offer alternative class suggestions as a make-up or a discounted private lesson. In the event a class must be canceled a second time (consecutively), a make-up class will be scheduled.

  • I understand that tuition will not change for months with school closures and holidays/breaks.

Youth Company & C.Glass Dance Co Performance Company 

I understand that if I register for performance company I agree to following: 

  • Audition’s are required to perform in Company. 

  • Company tuition is made in addition to regular class tuition billed on the 1st of the month and is non-refundable. 

  • Rehearsals are required to be attended every Monday. Absences are allowed but if more than 3 consecutive absences are made, dancer may be subject to removal from Company and/or lose role in performance. 

  • Dancers are expected and it is mandatory to attend all rehearsals the week of the Performance as outlined on our season calendar. 

Property 

Students and their guests agree to respect and care for the Haus of Glass Dance property and equipment. This includes utilizing property/equipment appropriately, keeping personal belongings tidy, and cleaning up after oneself. I understand that students and/or guests of students that cause significant damage, will be held financially responsible for repair. Additionally, in order to extend the life of the Studio floors, students should only consume food and drink (except water) in the lobby and remove non-dance shoes prior to walking on studio floors (to avoid bringing in unwanted/unsafe material, such as rocks).

Visitor Agreement

I agree to consider the following: We would appreciate those not taking class (i.e., parents, family members, friends, etc.) to limit their time in the lobby during evening classes. Our lobby space is for students and staff for class prep, homework, and to take breaks comfortably. Students and staff really appreciate your understanding and consideration. Our student/staff bathroom is located inside our studio, unfortunately the bathroom will not be open to all families and parents unless in an emergency. There is a restroom located inside the main building on the second floor for you to use. If the main building is locked, please ask a staff member at the front desk for the bathroom key.

Photo/Video

I am aware that Haus of Glass Dance  films and takes photos for promotional purposes throughout the year. I grant Haus of Glass Dance, its representatives and employees the right to film and take photographs of me, my child and/or my property in connection with these efforts. I authorize Haus of Glass Dance, copyright, use and publish the same in print and/or electronically. I agree that Haus of Glass Dance  may use such video footage and photographs of me and/or my child with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and Web content.

Return/Refund Policy

Refunds There are no refunds for tuition, registration fees, recital tickets, performance fees, private lesson packs or any other tuition related fees. 1 month in advance notice must be made in order to cancel your class tuition or membership. Ex: If you want to cancel November tuition you must notify us by October 1st, otherwise, November tuition will be pulled and your classes/membership will be cancelled for December. If no communication is made and tuition pulls, no refunds or credits will be granted.

Returns
Any merchandise can be returned for exchanged without damage We make returns easy! Online purchases of dance wear clothing only are valid for a refund within 14 days from purchase. Items must be returned unwashed, unworn, undamaged. All returned items must be accompanied by the order confirmation receipt. Gift cards, e-gift certificates, tights, shoes, water bottles, stickers, and masks may not be returned. If returned, no refund will be given.
